Free Baptism Program, for LDS Primary, Editable

I was asked to make a simple baptism program for a friend, and I thought...I'm going to put this on my blog for anyone who needs it.
You've got to remember not to print these, but to download them to your computer so you can edit it. It will be in a PDF format, so you should be able to do it with any program that can take that format.
If you can't figure out how to edit it, you can email it to your favorite copy center and I'm pretty sure they will be able to do it. Just be sure to compress it and keep it in PDF.
